Licensed Clinical Social Worker (LCSW) Care Advocate - Remote, United States (1099 Contract)

About the Role

This is an opportunity to use your social work expertise where it matters most: the space between a member's medical care and the rest of their life. As an LCSW Care Advocate, you support Carewell members, many of them older adults and caregivers, whose health is shaped by housing, finances, isolation, family dynamics, and access to services.

The role is built around three connected areas:

• Psychosocial Assessment — identifying the social, emotional, and practical barriers that affect a member's health and wellbeing.
• Resource Navigation — connecting members and caregivers to benefits, community services, and supports that actually fit their situation.
• Behavioral Health & Caregiver Support — offering brief, in-scope support and making sure members get connected to the right level of ongoing care.

This is a 1099 contract engagement. You set your own schedule and can work part-time, full-time, or anything in between. All member interactions take place by phone or video. This role is focused on advocacy and navigation and does not include ongoing psychotherapy.

What You'll Do (Scope of Services)

Assessment & Planning

• Complete psychosocial and SDOH assessments covering housing, food security, finances, transportation, safety, caregiver strain, and social isolation.
• Build member-centered plans with clear, realistic goals, and update them as circumstances change.
• Screen for depression, anxiety, cognitive concerns, and safety risks using standardized tools.

Resource Navigation

• Help members apply for and access benefits such as Medicaid, Medicare Savings Programs, SNAP, and utility or housing assistance.
• Connect members to community resources, including Area Agencies on Aging, meal programs, transportation, and in-home support.
• Follow up to confirm connections were made and resources are working, not just referred.

Behavioral Health & Caregiver Support

• Provide brief, solution-focused support within your scope, and coordinate referrals to behavioral health providers when ongoing care is needed.
• Support family caregivers with education, planning, and connection to respite and support resources.
• Identify crisis situations and follow escalation protocols immediately, including same-day routing of safety concerns.

Documentation & Compliance

• Document every member interaction accurately and within the timeframes set in your services agreement.
• Follow consent, privacy (HIPAA), and mandated reporting requirements on every interaction.

• Resource and benefit connections completed and confirmed for the members you support
• Member and caregiver-reported experience

Quality & Compliance

• Documentation complete, accurate, and on time
• Safety and crisis concerns escalated the same day they are identified

Who You Are

Required

• Active, unrestricted LCSW license in at least one U.S. state.
• Master of Social Work (MSW) from a CSWE-accredited program.
• Located in the United States.
• 2+ years of post-master's experience in medical, community, geriatric, or healthcare social work.
• Strong working knowledge of public benefits and community resources, and where members get stuck.
• Demonstrated ability to build trust with older adults and caregivers, including those who are skeptical or overwhelmed.
• Comfortable with care management platforms and phone/video communication tools.
• Self-directed. You can manage your own member load and schedule independently.
• Your own reliable computer, high-speed internet, and a private, HIPAA-appropriate workspace.

Nice to Have

• Experience with Medicare, dual-eligible, or older adult populations.
• Hospice, palliative care, dementia, or caregiver support experience.
• Licensure in multiple states.
• Bilingual (Spanish preferred; other languages depending on member population).
