ESSENTIAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are to be performed in accordance with CSI’s/Spark policies, procedures, guidelines, contractual commitments, and regulations.
- Identify and qualify potential clients.
- Assess patient needs and develop individualized care plans.
- Cultivate and maintain strong relationships with referral sources, i.e., discharge planners, physician offices etc. to expand our client network.
- Coordinate care with other providers, such as physicians, nurses, social workers, and therapists.
- Advocate for patients' rights and interests.
- Educate patients and families about their health conditions and treatment options.
- Monitor patient progress and adjust care plans as needed.
- Manage patient records and documentation.
- Track patient progress and outcomes.
- Participate in quality improvement initiatives.
- Provide clinical expertise with our sales representatives.
- Accompany sales representatives on sales calls as needed to offer clinical support and reinforce messaging.
- Conduct presentations and site tours for prospective clients and referral sources.
- Function as a trusted resource for sales representatives, potential clients, and patient families
- Deliver exceptional customer service to team members, patients, and patient families.
EDUCATION AND/OR EXPERIENCE:
- Bachelor's Degree (BSN) from an accredited nursing program.
- Minimum of 2 years of experience as an RN.
- Experience caring for pediatric patients.
- Case Management or Care Coordination experience preferred.
- Experience in sales or marketing is a plus.
CERTIFICATES, LICENSES, REGISTRATIONS:
- Current Texas RN license
- BLS Certification is required.
OTHER SKILLS AND ABILITIES:
- Bilingual-English & Spanish is required.
- Knowledge of home healthcare regulations, policies, and procedures.

POSITION SUMMARY: The Registered Nurse Navigator is responsible for providing comprehensive care coordination and case management services to patients with complex medical needs who may also be experiencing social, emotional, or financial challenges. This includes creating and fostering relationships with discharge planners and the patient’s interdisciplinary healthcare team, assessing the patient’s needs, developing care plans, coordinating care with other providers, and advocating for patients' rights.
As an RN Navigator, you will play a crucial role in bolstering our sales representatives' efforts as they engage with prospective clients and referral sources. PHYSICAL DEMANDS:
- Hybrid Role. This position requires some remote work from home with occasional travel up to 75%, requiring peripheral and depth perception for safe operation of a motor vehicle, to the Houston area.
- The ability to sit for extended periods in a motor vehicle and walk reasonable distances to client meetings, depending on the schedule.
- Regular use of hands for writing, operating a computer, or using a phone.
- Frequent reaching with hands and arms to complete office tasks.
- Rarely, the position may require that the employee stoop or kneel.
- You must be able to hear and speak for the use of the phone, meet with existing and potential clients, exchange information both face-to-face and over the phone or other medium, and conduct presentations and employee training.

- Due to the travel requirements of the position the employee will frequently be exposed to outside weather conditions.
